School Entry Health and Wellbeing Questionnaire

When children start school in September, an online questionnaire will need to be completed for each child that attends a Lewisham school. This can be accessed through the online Thomson SchoolScreener® Portal or using the QR Code.

Please note that you can still access the school entry questionnaire throughout the year.
 

Screening

Every year in England, the School Health Service offers hearing and vision screening to reception year children. We also provide the National Child Measurement Programme (NCMP) for reception and year 6 children.

In Lewisham, checks are carried out by School Health Service staff (SCPHN nurses, staff nurses and health care assistants) using the Thomson SchoolScreener®. The programme is delivered in line with national and local guidance.
 

Hearing and vision: reception year only

Screening will take place at your child's school and you can access their results through the secure online Parent Portal.

If you are happy for your child to have their health checks you do not need to do anything. If you do not want your child to be screened, please opt out via the Parent Portal, contact us by email at lg.shsscreening@nhs.net  or call us on 020 3049 3415. Children will not be made to take part if they do not want to.

This screening may help in picking up any hearing or vision problems early, and we can provide you with advice and support to help avoid any impact on your child’s education. Referrals will be made for every child that fails hearing and vision test.
 

National Child Measurement Programme (NCMP) (Reception and Year 6 Only)

The National Child Measurement Programme (NCMP) is an important part of our local area’s work to promote healthy weight in children. 

It is important to have a good understanding of how children are growing, or if they are experiencing any hearing or vision problems early on so that advice and support can be provided.

If you do not want your child to be screened, please opt out via the Parent Portal, contact us by email at lg.shsscreening@nhs.net  or call us on 020 3049 3415. Children will not be made to take part if they do not want to.
 

Accessing your child’s screening results

Depending on your child’s results, we may contact you to discuss them in more detail. You can view your child’s results by logging on to the secure online Parent Portal. Your child’s school should provide you with a link to access this.

There are apps to automate vision and hearing screening, record a child's height and weight data, complete health needs questionnaires and immunisation programmes.

This online platform can help to speed up communication between you and healthcare professionals involved in your child's care. It's a quick and easy way to share consent, access test results and get up to date public health information.
