Bereavement Services at University Hospital Lewisham
The Bereavement Officer at University Hospital Lewisham is informed of all deaths and is able to help bereaved family and friends through the immediate administrative process following a death.

Please contact the Bereavement Officer on 020 8333 3431 between 10am and 12 noon, Monday to Friday, before making an appointment to register the death.
What you will need
A Medical Cause of Death Certificate is required in order to register the death. A hospital doctor will issue the certificate and the Bereavement Officer will let you know when this is available.
The necessary papers will be processed as quickly as possible. If there is any reason for a delay the Bereavement Officer will explain this to you.

Additional support
Lewisham Council is also providing a service through their registrar in order to make things easier. This service means that you can just inform them that someone has died, and they will let all the necessary government departments and local authority services know. To use this service you will need to fill in a form on the GOV.UK website.
