This is a profoundly important report from which the NHS must learn. We are carefully considering each recommendation in detail, and will enact any necessary changes promptly both within our own organisation and working with partners across the whole health system.

The safety of our patients is the number one priority of the Trust. We have an honest and open culture and encourage our staff to highlight any patient safety or quality concerns that they may have.  We take any such concerns very seriously, and will always act immediately to investigate and address them.

Changes and improvements are strongly driven by patient and staff input. Patients and staff contribute ideas that will make services and care better for patients.  We will be looking for ways to strengthen both patient and staff input in an open and transparent way and extending the way we publish our feedback, not just on the wards but on our website and in our publications.

We will be publishing updates about our progress and the Trust also has a Patient Advice and Liaison Service (PALS) that provides a confidential advice and support service to help members of the public address any concerns they may have about the care we provide.
